For non-routine data requests:
1. Please complete and sign our fillable Data Request form and fax it to (804) 864-7983.
Upon review of the form, we will contact you regarding the status of your request.
PLEASE ALLOW AT LEAST ONE BUSINESS WEEK FOR THE COMPLETION OF A STANDARD DATA REQUEST AND/OR REVIEW OF THE NON-ROUTINE DATA REQUEST FORM.
