VDH Help

PEMS displays an on-line help feature at the top of each page. If you click on this icon, you will see an information display about the page, section or variable in question. The VDH PEMS web page also contains the SDN manual, data variables and definitions, data collection forms and quick tips.

If additional assistance is needed, please contact a VDH staff prior to contacting the CDC help desk... This will help ensure that problems that can be solved locally will not back log the CDC help desk.

Routine questions may be directed to your contract monitor. If you need more in-depth assistance, please contact Ayana Andrews-Joseph, the VDH PEMS Administrator, at Ayana.Andrews-Joseph@vdh.virginia.gov or 804-864-8017.

If you are experiencing a technical function problem/error with the PEMS software that cannot be resolved by the VDH PEMS administrator, CDC will want you to send a Program Change Request (PCR) by email or fax to document the problem.
For security reasons, the DHAP Help Desk cannot accept e-mail attachments or screenshots. Please do not, under any circumstances, send screenshots or any other client-identifying information. To submit a PCR, e-mail or fax the DHAP Help Desk and include the following information/descriptions in body of your e-mail or fax:

  • Description of error (one line about the error)
  • User name, phone number, email address
  • Agency name
  • Name of page on which error occurs
  • Actions leading to error (please include detailed information)
  • Type of data entered on page that error occurred
  • Exact error message
  • Can you duplicate the error?
