What is WIC?
WIC is a Special Supplemental Food Program for Women, Infants, and Children.
Who is eligible for WIC?
Women:
Pregnant, Post Partum or Breast-feeding
Infants:
Under one year of age
Children:
Under five years of age – once turns five years old, he/she is no longer eligible for the WIC Program.
All applicants must qualify both financially and nutritionally.
What you need to bring to your appointment
- Infants must be physically present at certification
- Your child (over the age of one) or a medical form that is no older than 60 days, from a doctor which includes a height, weight and hemoglobin/hematocrit.

- TANF
- Medicaid Card
- School Lunch Notice
- CMSIP
- Current Paystub/W-2
- Proof of identification
- Social Security Card
- Medicaid Card
- Driver’s License
- Birth Certificate
- Crib Card
- Proof of residency (911 & Mailing Address)
- Driver’s License
- Rent/Mortgage Receipt
- Medicaid Card
- Utility Bill
- WIC folder (Recertification Only)
- Immunization record
