Death Records
Death Certificates are available from the Prince William Health District for individuals who have died in Prince William County (including the towns of Dumfries, Haymarket, Occoquan, and Quantico), Manassas or Manassas Park. Death Certificates are retained for five years in the Prince William Health District. After the five year period you must contact the Virginia Department of Health, Office of Vital Records, at 804-662-6200 for certified copies. Death Certificates are available to immediate family members only. Immediate family members are defined as mother, father, husband, wife, child, brother, sister, grandparent, grandchild, or great-grandchild with a valid picture ID. All others can receive a Verification of Death document that will show the name of the decedent, date of death, date of birth, and the last four digits of the social security number. The Verification of Death will not show the cause of death nor the address where the death occurred. The charge is $12.00 per certified copy for both Death Certificates and Verifications of Death. Cash and check are the only methods of payment accepted for this service. We are not able to process credit cards for a Death Certificate or Verification of Death.

Birth Records
Birth Certificates are available only from the Virginia Department of Health, Office of Vital Records. Birth Certificates are provided to immediate family members only. Immediate family members are defined as mother, father, husband, wife, child, brother, sister, grandparent, grandchild, great-grandchild with a valid picture ID.
