The Health Department is the principal deputy registrar of vital
statistics for the City of Hampton. This includes registration
of all births, deaths, and fetal (baby) deaths in the City of
Hampton. Copies of death certificates for deaths occurring in
the City of Hampton within the past five years are certified upon
request at a cost of $12.00 each. Copies of death certificates for deaths occurring in the
City of Hampton prior to the past five years are issued from
the Vital Statistics Office in Richmond, Virginia. Copies of
birth certificates are also issued from the Vital Statistics
Office in Richmond, Virginia. Declaration of Paternity forms
are notarized upon request of the applicant.
