Information regarding fees for health department services can be found below. Services that we normally provide will not be denied because of the inability to pay. All private health information will be kept in accordance with HIPPA. Any administrative or billing questions should be directed to your local health department.
Dental services (therapeutic and preventive care for children only)
Proof of Identity
Social Security card
Driver's license
Medicaid card
For an infant or child proof may be newborn's hospital bracelet, foot prints, hospital crib card, birth certificate, Social Security card, or Medicaid card.Income tax return or W-2 form.
Proof of Income
Pay stub showing year-to-date 3 months worth of pay stubs
Income tax return or W-2 form
Medicaid card
Written statement from employer
Support letter (letter showing that someone provides you with food and shelter)
Statement of Social Security of income received
Statement from employment office of unemployment or of benefits you are receiving
These are services offered by the local health department at no cost. 
Cost for these services will be determined by the health department’s eligibility process.
